- The request must include the name-based check form, payment authorization and correct fee.
- BCI does not process emailed requests.
- Requests may be mailed, hand-delivered or faxed as permitted by the current instructions.
- BCI does not return results by telephone, fax or email.
- A waiver is not always legally required, but without one, an arrest more than 12 months old without a disposition cannot be released to a non-criminal-justice agency.
- The waiver signature must be dated within 180 days of submission.
